UHBW Subject Access Request (SAR) Portal for Access to Health Records

Welcome to the UHBW SAR Portal

The UK General Data Protection Regulation 2021 (UK GDPR 2021) read together with the Data Protection Act 2018 (DPA 2018) gives you the right to access the information we hold about you or others, providing you have a legal right of access.

There is no charge, but UHBW reserves the right to make an administrative charge in the case of requests which are manifestly unfounded or excessive.

The UHBW SAR Team is not obliged to comply with any request unless we receive sufficient documentation to identify the patient or applicant (if different), to locate the information held and to confirm if the applicant has sufficient right of access. By making this application you confirm the information given is correct and you are entitled to apply for access under the DPA 2018 and UK GDPR 2021. You are also confirming that it is necessary for the UHBW SAR Team to confirm your identity, right of access and for the Team to request more detailed documentation from you if required.

Once your request has been submitted via the Portal the UHBW SAR Team will send your request to the appropriate healthcare professional for their confirmation that it is safe to disclose the health records. This is a legal requirement under data protection law. The healthcare professional may recommend the best way for you to view the health records; 1) copies sent to you directly which is the most common form of disclosure 2) by making an appointment with the healthcare professional or 3) viewing the health records in the Health Records Department 

We aim to disclose health records within one month from receiving your request and sufficient documentation. This may be extended in complex cases however we will inform you of any new timescales.

In line with other healthcare providers, we are managing unprecedented challenges arising from COVID-19. The Trust seeks to prioritise clinical care which is severely impacting upon the availability of healthcare staff to assist with Subject Access Requests. We aim to disclose health records within one month however we are currently experiencing significant delays of up to three months due to staff having to self-isolate and the demands on admin/clinical care across the Trust.
